   Eligibility Requirements for both Administrative and Clinical Staff:

  • Potential Employees MUST be able to pass State & Federal Background Checks

  • Must be eligible for employment in the U.S., proof of eligibility & identity are required with 2 types of ID.

  • Tuberculosis Test (within 12 months) or Chest X-Ray (within 24 months with current note of clearance from Physician)

 

   Additional Training requirements for Clinical Staff:

  • Active CPR & 1st Aid Certifications;

  • Active Certified Nursing Assistant (CNA) Certificate

  • Active Registered Nurse (RN) License

  • required state trainings
